| dc.description.abstract | Objective: The aim of this study was to determine the distribution of homeostatic model assessment of insulin resistance (HOMA-IR) values and define its cutoff associated with metabolic syndrome (MetS) in the participants of the Study of Cardiovascular Risk in Adolescents (Estudo de Risco Cardiovascular em Adolescentes). Methods: MetS was defined according to the International Diabetes Federation criteria. HOMA-IR values were calculated and tabulated by corresponding percentiles for age and sex. Receiver operating characteristic curves were constructed to identify the optimal cutoff values of HOMA-IR associated with MetS in the total population and by sex. Results: We evaluated 37 815 adolescents ages 12 to 17 y. The highest HOMA-IR medians were found among girls and boys ages 12 and 14 y, respectively. Thereafter, values tended to decrease with age. The optimal cutoff values of the HOMA-IR associated with MetS in the total population, in female adolescents, and in male adolescents were 2.80, 2.32, and 2.87, respectively. Insulin resistance was prevalent in 19.1% (95% confidence interval, 17.7-20.7) of the total population, and the prevalence was higher among girls and overweight Brazilian adolescents. Conclusions: These findings may serve as new reference points for detecting insulin resistance in Brazilian adolescents. | en |
| dc.description.abstract | Objetivo: O objetivo deste estudo foi determinar a distribuição dos valores do modelo homeostático de resistência à insulina (HOMA-IR) e definir seu ponto de corte associado à síndrome metabólica (SM) nos participantes do Estudo de Risco Cardiovascular em Adolescentes (Estudo de Risco Cardiovascular em Adolescentes). Métodos: a SM foi definida de acordo com os critérios da Federação Internacional de Diabetes. Os valores de HOMA-IR foram calculados e tabulados pelos correspondentes percentuais para idade e sexo. Curvas características operacionais do receptor foram construídas para identificar os valores de corte ideais do HOMA-IR associados à SM na população total e por sexo. Resultados: Foram avaliados 37 815 adolescentes de 12 a 17 anos. As maiores medianas do HOMA-IR foram encontradas entre meninas e meninos de 12 e 14 anos, respectivamente. Posteriormente, os valores tenderam a diminuir com a idade. Os valores de corte ótimos do HOMA-IR associados à SM na população total, em adolescentes do sexo feminino e em adolescentes do sexo masculino foram 2,80, 2,32 e 2,87, respectivamente. A resistência à insulina foi prevalente em 19,1% (intervalo de confiança de 95%, 17,7-20,7) da população total, e a prevalência foi maior entre meninas e adolescentes brasileiros com excesso de peso. Conclusões: Esses achados podem servir como novos pontos de referência para a detecção de resistência à insulina em adolescentes brasileiros. | pt |
